Retatrutide is an experimental peptide that acts on the GIP, GLP-1 and glucagon receptors. It is not FDA approved, and FDA says it cannot be used in compounding. It has been studied for obesity, type 2 diabetes, knee osteoarthritis pain and sleep apnea in phase 2 and phase 3 trials [1, 6, 7, 8, 9].

## Key facts

- Class: Triple agonist of GIP, GLP-1 and glucagon receptors (investigational) [1, 3]
- Amino-acid length: 39 amino acids, built on a GIP backbone, with a fatty side chain [4]
- Half-life: About 6 days (phase 1b study) [2]
- Route used in studies: Under the skin once weekly in studies [1, 6]
- First studied: 2022 (first discovery and human trial reports, as LY3437943) [3, 2]

## How it works

Retatrutide is one peptide built to act on three hormone receptors at once. It binds to the receptors for GIP, GLP-1 and glucagon [1]. GIP and GLP-1 are gut hormones released after eating. Glucagon is a hormone made by the pancreas [3].

In lab tests, retatrutide showed balanced activity at the glucagon and GLP-1 receptors and stronger activity at the GIP receptor [3]. In obese mice, the glucagon part added a rise in energy use on top of the lower food intake driven by the GIP and GLP-1 parts. Together these lowered body weight [3].

The peptide is a chain of 39 amino acids based on the GIP sequence. A fatty acid is attached so that it binds to albumin in the blood, which slows its clearance [4]. A phase 1b study in people with type 2 diabetes found a half-life of about 6 days, which supports once-weekly dosing [2].

Because it is still experimental, these effects come only from research studies. FDA says retatrutide has not been found safe and effective for any condition [9]. In the phase 3 trials, it was tested against placebo, not against approved drugs [6, 7].

## What the research shows

## Doses used in studies

The phase 3 TRIUMPH trials gave 4 mg, 9 mg or 12 mg once weekly for 80 weeks [6, 7]. The phase 2 obesity trial tested 1 mg to 12 mg once weekly for 48 weeks, with some groups starting at 2 mg or 4 mg and stepping up [1].

The phase 1b study tested weekly doses from 0.5 mg up to a stepped schedule ending at 12 mg [2]. There is no FDA-approved dose, because retatrutide is not an approved drug [9].

**Dosing is set by a licensed provider.**

## Side effects and safety

In trials, stomach side effects were the most common. In the phase 2 obesity trial they were dose-related, mostly mild to moderate, and less common with a lower starting dose. Heart rate rose with dose, peaked at 24 weeks and then declined [1].

In TRIUMPH-2, diarrhea occurred in 27% to 34% of retatrutide groups vs 13% on placebo, and nausea in 14% to 28% vs 8%. Low blood pressure and dysesthesia (abnormal skin sensations) were more frequent than with placebo. Stopping for side effects or death was 12% with 9 mg and 8% with 12 mg, vs 5% with placebo [7].

FDA warns that products sold as retatrutide are unapproved drugs. FDA says retatrutide cannot be used in compounding under federal law, is not a component of any FDA-approved drug, and has not been found safe and effective for any condition [9].

## Legal and regulatory status

- 2026-10-01: FDA's GLP-1 concerns page states that retatrutide cannot be used in compounding under federal law and is not a component of any FDA-approved drug. It lists warning letters to companies marketing unapproved drugs such as retatrutide [9]. [Source](https://www.fda.gov/drugs/drug-alerts-and-statements/fdas-concerns-unapproved-glp-1-drugs-used-weight-loss)
- 2026-05-14: Retatrutide does not appear in any category of FDA's list of bulk substances nominated for 503A compounding (list updated May 14, 2026) [10]. [Source](https://www.fda.gov/media/94155/download)
- 2026-01-01: The WADA 2026 Prohibited List took effect. Its S0 class bans drugs in clinical development with no approval for human use. WADA does not name retatrutide, so its status rests on that general wording [12]. [Source](https://www.wada-ama.org/sites/default/files/2025-09/2026list_en_final_clean_september_2025.pdf)
